Editorial : Emerging treatments and approaches for moral injury and moral distress

Current evidence-based therapies to treat trauma-affected populations, especially military members and first responders, have had variable success. Treatment response may be impeded by a lack of clinical attention to moral aspects of psychotrauma.

Exposure-based treatments for childhood abuse-related post-traumatic stress disorder in adults : a health-economic evaluation

Background: Prolonged exposure (PE) is an effective treatment for post-traumatic stress disorder (PTSD).

 

Objective: This study aimed to analyse the cost-effectiveness of three exposure-based treatments in patients with childhood abuse-related PTSD.

Het algemene doel van dit proefschrift was het verkennen en evalueren van nieuwe en methodische manieren om de langdurige behandeling van patiënten met een ernstige niet-psychotische psychiatrische aandoening, die door hun sociaal-psychiatrisch verpleegkundigen als moeilijk worden ervaren, te verbeteren. Daarbij was het belangrijk om inzicht te krijgen in waarom zij als moeilijk worden ervaren. Hoewel er verschillende antwoorden op de onderzoeksvragen zijn verkregen, komen er nieuwe en fundamentele vragen naar voren over de zorg voor deze groep patiënten. 

 

 

 

Epigenetics of traumatic stress : The association of NR3C1 methylation and posttraumatic stress disorder symptom changes in response to narrative exposure therapy

Epigenetic processes allow plasticity in gene regulation in response to significant environmental events. Accumulating evidence suggests that effective psychotherapy is accompanied by epigenetic changes, rendering DNA methylation a potential biomarker of therapy success. Due to the central role of glucocorticoid dynamics in stress regulation and the alteration of aversive memories, glucocorticoid receptors are likely involved in the molecular processes that are required to successfully treat Posttraumatic Stress Disorder (PTSD).

 

The impact of trauma-focused psychotherapy for posttraumatic stress disorder on interpersonal functioning : A systematic review and meta-analysis of randomized clinical trials

Interpersonal functioning is a common concern for people with postttraumatic stress disorder (PTSD) but is not a key target of most trauma-focused psychotherapies (TFPs). We preregistered and undertook a systematic review and meta-analysis of randomized clinical trials (RCTs) examining the efficacy of TFPs for improving interpersonal functioning. Studies were identified through the PTSD Trials Standardized Data Repository, scholarly databases, and the solicitation of unpublished data from the PTSD research community following current PRISMA guidelines.

 

Dialectical behaviour therapy for posttraumatic stress disorder (DBT-PTSD) : transportability to everyday clinical care in a residential mental health centre

Background: Dialectical Behaviour Therapy for Posttraumatic Stress Disorder (DBT-PTSD) is a phase-based treatment for PTSD. The DBT-PTSD treatment programme’s efficacy has not been tested during standard operation, outside of laboratory outcome studies.

 

Objective: The present pilot study investigated the transportability of the DBT-PTSD treatment to a real word clinical setting in a residential mental health centre.

Trauma is associated with an increased likelihood of experiencing suicidality, indicating the need for and potential value of trauma-informed suicide prevention strategies.

 

Toward staging differentiation for posttraumatic stress disorder treatment

Objectives: Several medical and psychiatric disorders have stage-based treatment decision-making methods. However, international treatment guidelines for posttraumatic stress disorder (PTSD) fail to give specific treatment recommendations based on chronicity or stage of the disorder. There is convincing evidence of a finite range of PTSD symptom trajectories, implying that different phenotypes of the disorder can be distinguished, which are highly relevant for a staging typology of PTSD.

 

Hair Cortisol in Service Dogs for Veterans with Post-traumatic Stress Disorder Compared to Companion Dogs (Canis Familiaris)

Service dogs are trained to assist humans. This assistance potentially exposes them to stressors To investigate if service dogs are exposed to more stressors than companion dogs we questioned whether hair cortisol levels differed between both groups.

 

We studied this by cutting a tuft of hair from the neck of 19 companion and 11 service dogs. Cortisol levels were subsequently analyzed via immunoassay and compared via a simple linear regression model. The influence of coat color, season, sex, other dogs, pets, or mental health diagnoses in the household was also checked .
